Register as a patient
Firstly you will need to speak to a member of the reception team to ensure you are living within our practice area. If so you will then need to complete a GMS1 form which you can obtain from reception or you can download here. When you return your forms to reception you will be asked to complete a new patient questionnaire and will be invited to book and appointment with our Nurse Associate for a healthcheck. We offer a complete range of services.
Make an appointment
Routine appointments can be booked up to six weeks in advance either via patient access or speaking to reception in person or calling 01384 366166. If you have a medical condition that is urgent or acute and you need to be seen the same day please speak to a member of the reception team.
Request a home visit
If you are too ill to attend the surgery, please request a home visit via the appointments telephone line. Our clinicians will assess your symptoms over the phone prior to arranging a visit.
Please refrain from requesting a home visit unless absolutely necessary, they take up a great deal of the doctors time and should only be reserved for patients who are medically unable to attend the surgery. Transport problems will not be accepted as a reason for the doctor to do a home visit. By attending the surgery the doctor has the necessary specialist equipment available to examine you, with the help of other health professionals if necessary. This is to reduce the impact on other patients.
If you need to request a home visit please try and ring before 11am to enable the doctor to see you after morning surgery.
No patient in definite need of a home visit will be refused.
Order a repeat prescription
Please note that 72 hours is required to process your repeat prescription request. You can order your prescription by registering for online patient access, by email to rrs.reception@nhs.net, in writing using the right hand side of your last prescription or completing an order form which you will find in reception. Telephone requests for repeat prescriptions will be taken between 11am – 4pm for housebound patients only
Obtain results
Please telephone between 11am – 4pm and a member of the reception team will be able to advise if your test result is back and any comments made by the doctor.

Make a complaint
