Patient Surveys & Audits

Following implementation of the 2015 action plan we have seen that with the advertisement of online services along with a guide on how to register that there has been an increase in registration for this service. Numbers are growing especially as we are now live with EPS and patients are finding this service extremely useful. The waiting room holds a host of information on preventative care and awareness that is now zoned into areas such as “Young famailes and children”, “seasonal information” and “young adults” making information more easliy accessible to our patients and we have used the Friends and Family Questionnaire as a way to promote services available to our patients such as minor surgery and smoking cessation.

National Diabetes Audit

We are taking part in an important national project about people at risk of diabetes and diabetes care and treatment in the NHS. The project is called the National Diabetes Audit (NDA).

If you have diabetes, non-diabetic hyperglycaemia, impaired glucose tolerance or pre-diabetes, your GP practice will share information about your diabetes care and treatment with the NDA. The type of information, and how it is shared, is controlled by law and enforced by strict rules of confidentiality and security.

For further information about how your data is used, please see the NDA patient information leaflet.

Taking part in the NDA shows that this GP practice is committed to reducing diabetes and improving care for people with diabetes.

If you do not want your information to be used, please inform the receptionist, your GP or nurse. This will not affect your care.

More information is available – please ask a member of staff for a patient information leaflet or click on the link below to view the National Reports
